Insurance: 

 

 Please be sure to bring your current health insurance card with you.  If you are an out of province patient, we will need to verify coverage before you see the doctor.

Additional Fees: 

 

There are additional fees for completion of forms for use outside of the patient's medical record, including:

  • Day Care

  • Sick Notes

  • Employment

  • Life Insurance.

  • Family Medical Leave

  • Health Insurance Forms

Will my insurance cover Athletic Therapy?

 

Many companies cover athletic therapy including:

  • Sun Life

  • Allsport Insurace Marketing

  • The Co-Operators

  • Canadian Hockey Assn.
