Getting started with Botox treatment at Bayside Medical Centre

You have decided to start Botox treatment for your migraines. The first step is getting a Special Authorization from your extended medical insurance plan.

Dr. Graboski will enroll you in the Abbvie (Botox) Patient Support Program. They are there to help you with the application process as it can be a little confusing. They are helpful and know the ins and outs of the application process.

Once they have your Special Authority form ready with as much information as they with your help can fill out, they will fax it to Dr. Graboski so she can review it. 

There is an admin fee of $75.00 + tax for initial applications to your extended medical and a fee of $50.00 + tax for renewals which is paid by the patient as there is no MSP code for Physicians paperwork. 

We will call you to let you know it has been faxed and to collect the fee via Visa, Mastercard or Etransfer to baysidemedcentre@gmail.com.  If you prefer to send a cheque or come in to pay with Debit that is fine as well.

 Your insurer will NOT let us know when you are approved. They will let you know and then you will need to call us so we can make your appointment for our next Botox clinic. Once approved, most insurers authorize Botox treatment for a set period and then you need to reapply.

The Brentwood Pharmasave will reconstitute the Botox on the day of your appointment and deliver it to the clinic. Please make sure you are in touch with the Pharmacy at least 24 hours ahead to make sure your insurance covers the full cost and if not pay your portion. If the Pharmacy cannot be in touch with you, they do not deliver your Botox and your appointment may be canceled. Phone # 250-652-1235

There is an injection fee of $200.00 that is not covered by MSP. This is charged at the clinic at each Botox appointment. Dr. Graboski sets up your treatment plan and then Dr. Graboski or a Registered Nurse will perform your injections.

If Dr. Graboski is away she will try to have a locum physician. You can have a follow up scheduled with Dr. Graboski as needed.

You will be sent Headache Questionnaires 1 week ahead of each appointment through the Portal. Please fill them out as soon as possible as they are reviewed ahead of all appointments and are often required for insurance renewals.

We require 2 business days notice for any cancellations.

Your appointment time is reserved for you.  Injection days are very busy so if you are late, it can impact many patients.  If you are going to be late, please call the office.  If you are more than 15 minutes late, your appointment might be rescheduled.
